A thorough nutrition assessment is crucial for determining needs and care plan for patients. For this module, I completed documentation on nutrition related problems and conditions, and coordinated my caseload based on level of acuity. 

The Nutrition Care Process is used to assess, diagnose and determine appropriate plan of action for patient care. Using the NCP allowed me to formulate PES statements based on observed findings. The NCP also enabled me to engage with other members of the disciplinary team including doctors, nurses, speech and physical therapists, pharmacists and case managers.
